This pack includes six form options (three whole class and three individual student) for tracking pre- and post-assessment data on your choice of skills, standards, units, or writing pieces.
The applications for use can include:
-Tracking student growth for teacher evaluation systems based on Charlotte Danielson's Framework for Teaching (or your state's system/rubric).
-Conversations about instruction/assessment data in professional learning communities (PLCs).

These assessment data sheets provide a place to record your students’ performance on key kindergarten skills from quarter to quarter. Data sheets provided in this pack include assessments for the following skills/Common Core Standards:
-Uppercase letter identification (RF.K.1.D)
-Lowercase letter identification (RF.K.1.D)
-Letter sounds (RF.K.3.A)
-Number identification (0-20)
-Shape identification: 2-D and 3-D (K.G.A.2)
-Rote counting to 100 (ones and tens) (K.CC.A.1)
-Counting 20 objects (K.CC.B.5)
-Sight Word Identification (Reading Street List) (RF.K.3.C)
There is also a color coding key included to help organize your results from quarter to quarter.
Directions for use: Print and staple one copy of this packet for each student in your class. You will use only one copy for the entire school year, as the color code provided will help you track results from quarter to quarter. This packet is NO-PREP meaning that you can use the sheets to both assess the student AND record their results! You may consider keeping the packet in your student portfolio folders or data binders when not in use.
